Immunization Compliance


地中海+普氏 Immunizations Records FlyerLouisiana law requires that all students entering Grambling State University to be immunized for the following: Measles (2 doses), Mumps, Rubella--required for those born on or after January 1, 1957; Tetanus-Diphtheria (within the past 10 years); and against Meningococcal disease (Meningitis-2 doses). You will not be eligible to pay University fees until all immunization records are in compliance or the exemption 签署. Effective August 23, 2021, the U.S. Food and Drug 政府 (FDA) granted full approval of the Pfizer 新型冠状病毒肺炎 vaccine for people age 16 and over. 自去年12月以来 of 2020, the Pfizer vaccine has been available through Emergency Use Authorization, and more than 200 million doses have been administered in the U.S. 独自一人.

The University of Louisiana System previously requested that the Louisiana Department of Health (LDH), upon full approval by the FDA, add 新型冠状病毒肺炎 to the schedule of required immunizations for students at Grambling State University and other UL System institutions. LDH granted this request.

As a result of FDA approval, 新型冠状病毒肺炎 is now on the schedule of required immunizations for college students in Louisiana.

Accordingly, all students are encouraged to be vaccinated for 新型冠状病毒肺炎 as soon as 可能的. Documentation of immunization against 新型冠状病毒肺炎 must be provided prior to November 22, 2021 to register for the spring semester and all terms moving forward. Alternatively, students may provide one of the following:

  1. documentation from a physician of medical contraindication for the vaccine, or
  2. written dissent for taking the vaccine.

 

Immunization/TB Questionnaire (Mandatory)

Louisiana Law (R.S. 17:170/R.S. 17:170.1/Schools of Higher Learning) requires that all students entering Grambling State University to be immunized for the following: Measles (2 doses), Mumps, Rubella--required for those born on or after January 1, 1957; Tetanus-Diphtheria (within the past 10 years); and against Meningococcal disease (Meningitis-2 doses).

The student health center is evaluating all entering students for exposure to tuberculosis (TB). 

You will not be eligible to pay University fees until all immunization records are in compliance or the exemption 签署. If you waive immunizations, you may be required to leave school during a disease outbreak 在校园. 

 

All incoming students are required to use 地中海+普氏 to submit medical/immunization records.
